Part I: General Provisions (General Terms & Legal Disclaimers)

These regulations apply to all 2026 Health Promotion activities. Registration constitutes agreement to the following terms.

I. Purpose

This program aims to assist employees in establishing self-health management behaviors. It is a health promotion initiative, not an acute or invasive medical service, and does not constitute medical diagnosis or treatment advice.

II. General Eligibility

  1. Restricted to active employees of designated partner corporations.
  2. Participants must agree to comply with these regulations and the corresponding Annual Activity Program.
  3. The organizer reserves the right of final decision regarding auditing, screening, and admission of participants.

III. Taxation & Rewards

  1. Tax Reporting: According to the R.O.C. Income Tax Act, prizes valued at NT$1,000 or more must be reported as annual income. Non-residents residing in the R.O.C. for less than 183 days are subject to a 20% withholding tax on all prizes, regardless of value.
  2. Prize Obligations: To qualify for draws or subsidies, participants must complete performance self-assessments or upload designated questionnaires as required. Winners must provide a copy of their ID or tax-related documents; failure to do so constitutes a waiver of the prize.
  3. Right of Interpretation: Details regarding reward amounts and formats are specified in the Annual Activity Program.

IV. Device Usage & Liability

  1. Proper Usage: Participants must operate devices according to the user manual or contact us for help.
  2. Compensation: If borrowed equipment is returned damaged or with missing accessories, the participant must pay compensation according to the amount specified in the Annual Activity Program.
  3. Non-Return and Discounted Purchase Clause:The obligation to return rented equipment may be waived, or the option to purchase such equipment at a discounted price may be granted, only to participants who meet the specific reward criteria set forth in the Annual Activity Program.Participants who receive rented equipment free of charge under these conditions agree to comply with all applicable income reporting and tax regulations.

V. Compliance & Privacy

  1. Data Protection: Participant data will be processed in accordance with the Personal Data Protection Act.
  2. Insurance Disclaimer: This activity is not a benefit under group life insurance. The organizer is solely responsible for all matters, independent of the insurance company.

VI. Miscellaneous

  1. This document serves as the institutional basis; specific execution details are subject to the Annual Activity Program.
  2. In the event of any conflict between these General Provisions and the Activity Program, the provisions of the Activity Program shall prevail. In the event of any discrepancy between the Chinese and English versions, the Chinese version shall prevail.

Part II: Specific Activity Programs (Details & Incentives)

I. Case Tracking Health Management Program
  • Target: Sub-healthy individuals or those seeking lifestyle adjustment advice to improve their physical condition.
  • Quota: 10 persons.
  • Resources: Rental of BP monitor, scale, glucose meter, and 50 units of supplies.
  • Incentives:
    • Subsidy:Choose either professional consultation or an activity subsidy of up to NT$500 per month.If you choose the subsidy, upload a photo of your activity invoice (Tax ID: 90491533) with your post-program questionnaire. The subsidy will be provided as a voucher after the program ends.
    • Completion: Rented equipment becomes a gift (belongs to the participant) upon meeting minimum tracking standards.
    • Outcome: 20% indicator improvement: NT$3,000 voucher; Return to healthy range: NT$10,000 voucher.
  • Liability: Compensation of NT$6,500 applies if completion requirements are not met and equipment is damaged.
II. Pain Awareness Health Management Program
  • Target: Occasional pain consultation and lifestyle or stress management support.
  • Quota: 10 participants.
  • Resources: Free rental of brand-new health wearable devices, professional consultation, and guided movement instruction.
    • Product Inspection & Warranty:This activity provides brand-new wearable devices. If no issues are reported within three days of delivery, the product is deemed to have arrived in perfect condition. Subsequent issues will be handled according to the standard warranty process.
    • Borrowing & Returns: Participants are responsible for the safekeeping of the device. It must be returned within one month after the activity ends. No fees apply if the device, packaging, and accessories are returned intact and functional; otherwise, compensation is required per activity regulations.
  • Incentives:Participants who complete all stress and emotion tracking and the post-program questionnaire may keep the wearable at a discounted price. If not kept, the device can be returned at no cost if undamaged, reset to factory settings, and returned with original packaging and accessories.
    • To fulfill the requirement for “complete all stress and emotion tracking,” participants must maintain a Mood Journal at least once per week and complete the Mental Energy Assessment (GAD-7 + PHQ-8 questionnaires) at least once per month.
  • Liability: Compensation of NT$12,500 applies for damaged or lost devices.
  • Discounted Purchase Option: Participants may retain the device after program completion at a discounted price of NT$6,500.
III. Self-Health Management Program

1. Target Audience & Quota

  • Eligible Participants: Active employees of contracted enterprise policyholders and their dependents enrolled in the group insurance plan.
  • Program Capacity: Limited to 300 participants, including accompanying family members/dependents who register as a team.

2. Program Objectives

  • Focus Areas: Physical activity and weight management.

3. Rewards & Incentives (Multiple Rewards Applicable)

Participants are eligible to accumulate multiple rewards across the following categories:

A. Completion Subsidy (Limited Quota)

  • Criteria: Participants who successfully complete all 4 cycles of the program will receive a subsidy issued in the form of vouchers.
  • Subsidy Amounts:
    • Individual: NT$200
    • Team (Additional): NT$300
  • Weight Management Resources: BMI is above 27 (proven by a health check report)
    • Apply for a one-time online consultation with a nutritionist or personal trainer during the event.
    • A free rental of a weighing scale (Rossmax LS212-B) or a wearable fitness tracker (Xiaomi Smart Band 10). The organizer reserves the right of final selection for product models with similar functionality.

B. Team Incentives

  • Team Formation: 1 to 5 members per team.
  • Lucky Draw: Teams that successfully meet their designated calorie expenditure or BMI improvement targets will be entered into a lucky draw.
    • Prizes: Two winning teams will be drawn for each category, with a prize of NT$2,000 per person.
    • Special Bonus: If a winning team’s members successfully shift their BMI from outside the healthy range back into the healthy range, each qualifying member will receive an additional NT$2,000.
  • Re-rolled: Each team is limited to winning only one prize throughout the campaign. If a member of the same team is drawn in both raffles, the second draw will be voided and re-rolled.”

C. Individual Incentives

  • Lucky Draw: Individual participants who consistently meet their calorie or BMI targets across all 4 cycles will be entered into a lucky draw.
  • Prizes: 3 winners will be drawn, receiving NT$1,000 each.
  • Special Award (Direct Reward): Any individual participant whose BMI successfully shifts from outside the healthy range back into the healthy range will directly receive a NT$2,000 reward.

4. Professional Consultation

  • Service: Medical consultation services are available via the official Line OA.
  • Quota: Strictly limited to 30 sessions in total.

5. Equipment Liability & Compensation

  • Device Rental: Program equipment is provided on a complimentary rental basis.
  • Liability: In the event of damage to or loss of the rented device, a compensation fee of NT$2,000 will be charged to the participant.
